Referral & Payment Information
Fee-for-Service Practice
We are an out-of-network, fee-for-service clinic. This model allows us to provide longer, more thorough sessions and maintain a high level of individualized care.
We accept:
- HSA/FSA cards
- Patients receive a superbill for potential out-of-network reimbursement
- Many patients receive partial reimbursement depending on their plan
Workers’ Compensation
We accept workers’ compensation patients and work directly with case managers and adjusters for authorization and billing.
- We provide detailed evaluation reports and progress updates
- We communicate directly with case managers to streamline documentation
- Our practice bills according to the appropriate state fee schedule or contracted rate
Medicare
At this time, we are unable to see Medicare beneficiaries for physical therapy services, even if patients wish to self-pay. (This may change in the future; updates will be provided as regulations allow.)
How to Refer a Patient
Providers and case managers can refer patients by:
- Sending a referral or prescription via fax: 844-806-0117
- Emailing questions or documentation to: [email protected]
- Having the patient book directly online (if work comp, prior authorization needed)
- Including any relevant records such as previous imaging, specialist notes, or prior therapy documentation
We strive to make coordination simple. Referring providers receive timely communication, evaluation reports, and ongoing updates throughout the patient’s course of care.
We’re Here to Support You

We regularly work with both local patients and those traveling from out of town, especially for specialized care in concussion, persistent pain, autonomic dysfunction, and hypermobility. We are happy to coordinate short-term, intensive care plans when needed.
If you have questions about whether a patient is a good fit or would like to discuss a specific case, please don’t hesitate to contact us.
